I hereby consent to and authorize an artist/esthetician at Diya Brows and Beauty to perform the procedure(s) that I have voluntarily elected to undergo. I have been informed that this procedure will involve some pain and discomfort. I understand that I am responsible for letting the artist/esthetician know of my preferences, particular instructions regarding design, shape & usage of certain products, allergies, medications I am taking, skin conditions that might negatively react to the services I am getting, sensitivities and any or all other issues before the service. I understand that the artist can only work with the amount of hair I naturally have and that they might not be able to achieve the exact look that I want. I understand that there are no guaranteed results for the services performed. The results are dependent on the age, skin condition, lifestyle, and aftercare of the client. I may require further treatments of the treated areas to obtain the expected results at an additional cost. I understand that overly sensitive or teary eyes can affect the quality of the eyelashes I am getting. I also understand that the darkness and quality of henna or tint on the eyebrows can be diminished if there is any prior work done on the eyebrows such as permanent makeup or tattoos. I understand that certain conditions of the treatment areas will affect the outcome of the services performed. The results of the services performed may not be visible right away. I understand the post-treatment home care instructions. I understand how important it is to follow all instructions mentioned to me for post-treatment care. In the event that I may have additional questions or concerns regarding my treatment or suggested home product/post-treatment care, I will consult the esthetician immediately. I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically. I understand the procedure and accept the risks. All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I do not hold the artist/esthetician, responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed today. I understand that I am responsible for asking any questions, or voicing any concerns about the services that are being performed. I understand that should I have a complaint of any kind whatsoever, I shall immediately notify the salon. I accept full responsibility for any and all, present and future, medical treatments and expenses I may incur in the event that I need to seek treatment for any known or unknown reason associated with this procedure. All of the services performed, payments and deposits are non refundable. I authorize photographs of the work performed to be taken both before and after to be maintained in my personal file or social media accounts of the salon. Appointment check-in can only be done 15 minutes before or after the appointment time. I understand that the payment is non refundable and that a no call no show will cause a forfeit of my payment. Appointment rescheduling can only be done once by calling the salon. It is up to Diya Eyebrow Threading’s discretion whether or not to accommodate requests made by clients. I understand that during some circumstances, the artist that I prefer might not be available. In that case, I consent to having my appointment be rescheduled to another time or with another artist at my chosen time. I understand that due to issues out of Diya Eyebrows Threading’s control, my appointment might have to be rescheduled or cancelled without prior notice. I understand that I am responsible for all of my personal belongings and the actions of the people that accompany me for my service. I also understand that any children I bring with me are not to be left unattended and that I am responsible for all of their actions. I have read and fully understand this agreement and all information detailed above.
